Details about your insurance benefits, co-payment or co-insurance, preauthorization, and other specific information is usually outlined on your insurance card, or available by calling the number on the back of your insurance card. A fee is charged for each service. You will be required to pay the co-payment or co-insurance (co-insurance is a percentage of the fee allowed by your insurance company and varies by health plan) amount, and any deductible assigned to you by your healthcare plan. A co-payment amount for mental health or behavioral health services is typically listed on the back of your insurance card. A co-insurance amount is generally specified by the insurance company after a claim has been submitted.
